It doesn't look tight, the movement of the foreskin is pretty good. I think I would call it tight if your had to forcibly push the skin forward over the head or even squeeze the head to make it smaller and get the foreskin over but it glides back and forth really well.

that's a very normal foreskin. I don't know why your doctor thinks it's tight.
